1) Before filling out this application, be sure you have the following info from all team members:

• Email Address

• Shirt Size

• Grand Champion Division

• • NOTE: Kids must be between the ages of 6-12 years old to participate.​

2) You'll also need:

•  A Team Captain

3) Complete the form and click "Submit and Pay".​

4) Complete your payment through PayPal.

5) THAT'S IT! You'll receive a confirmation email, and a MPMFT team member will reach out to you to offer further guidance.
